The Authority is seeking a provider to deliver an integrated 0-19 (up to 25 where appropriate) Healthy Child Programme, encompassing Health Visiting, School Nursing and Family Nurse Partnership services across Shropshire. The service will support children, young people and families through a community-based, preventative model that promotes positive health and wellbeing outcomes, reduces inequalities and responds to local need through population health management and local intelligence.
